Inspector Notes

An initial subsidy health and safety (SHSI) inspection was initiated and concluded on 11/13/2023. There were 75 children present with 14 staff supervising and 3 administrators. Children were observed during snack time, individual play time, and teacher led activities. A total of 5 child records & 5 staff records were reviewed. The inspection started at 10 am and concluded at 11:50 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on documentation review and staff interview the center did not ensure staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: The director confirmed that the files for Staff #1 (date of hire 8/21/2023) and Staff Members 2-5 (dates of hire 9/21/2023) did not contain documentation of a TB screening prior to coming into contact with children.
Plan of Correction: It was a confusion in interpretation in reading the website and all screenings will be completed by 11/30/2023

Based on observation and staff interview the center did not ensure that cribs shall be provided for children from birth through 12 months of age during the designated rest periods.
Evidence:
1. In the 9?12-month-old infant room there were 2 pack `n plays being used for the 2 infants (11months old) in care on the date of the inspection. There were no cribs available.
2. In the 6?9-month-old infant room there were 5 pack `n plays being used for the infants enrolled. There were no cribs available.
Plan of Correction: Under religious exemption cribs were not required however subsidy does require them and cribs are now in place as of 11/16/2023
