Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 9/11/2024, as a part of the conditional licensure period. The on-site inspection began at 10:25 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 13. The inspector reviewed 5 children?s records and 2 caregiver records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #2, and #5, who had been in care for over 60 days.
Plan of Correction: Will ensure parents provide all mandatory documents.
Standard 8VAC20-800-120-B
(Repeat Violation) Records for assistants and substitute providers shall contain documentation that two or more references as to character and reputation as well as competency were checked before employment. Caregiver #2, employed for more than a month, did not have documentation of references.
Plan of Correction: Will ensure staff have references before start dates.
Standard 8VAC20-800-60-B
(Repeat Violation) An up to date record for each child enrolled shall be maintained by the provider.
Child #2 record was missing: one address and telephone number for one of the parents place of employment; telephone number of the child's physician; a 2nd emergency contact; and first day of attendance.
Child #3 record was missing: 2nd emergency contact; and first day of attendance.
Child #4 record was missing: a home address for one of the parents; name, address, and telephone number of one of the parent's place of employment; an address and telephone number for the 2 emergency contacts; and first day of attendance.
Child #5 record was missing: the name and telephone number for one of the parents place of employment; the address for one emergency contact and a 2nd emergency contact; and first day of attendance.
Plan of Correction: Will enforce parent's fill out forms to their entirety.
Standard 8VAC20-800-70-B
Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration. There were no parent acknowledgements available for Child #2 and Child #5.
Plan of Correction: Will ensure parents provide all mandatory documents.
Standard 8VAC20-800-760-A-1
First aid kits shall contain all required emergency supplies. The first aid kit did not have tweezers.

The required emergency telephone numbers shall be posted in a visible area close to the telephone. Emergency telephone numbers were not posted.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated. The menu that was posted was not dated.
