During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-790-180-B
(Repeat Violation) The vendor is required to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
The vendor did not have completed attendance records for 3/17/2025 and several days following that.
Plan of Correction: The vendor stated that some of her documentation was soiled and could not be salvaged while some of the attendance sheets contained incomplete sig in and sign out by the parents. The provider has reminded parents to sign their children in and out each day as the arrive and leave A note to remind them has been placed on the attendance clipboard.
Standard 8VAC20-790-220-B
The vendor is required to inform the department's representative as soon as practicable, but no more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Child #1 sustained an injury on 3/17/2025 and was as a result, was assessed at a medical facility that evening. The vendor was advised by the parent that child #1 had been seen by a doctor for his injury, but failed to report this injury to the department representative.
Plan of Correction: The vendor received conflicting information and assumed that the child had not seen by a medical professional following his injury. In the future, if there is any doubt as to whether medical care was obtained, the provider will report the injury and circumstances to her inspector.
Standard 8VAC20-790-250-B
Caregivers and assistants must complete the Virginia Preservice Training for Child Care Staff sponsored by the Department of Education within their first 90 days of employment.
Staff #3 and staff #5, who have been employed for just under 1 year, did not have documentation to demonstrate that they had completed the Virginia Preservice Training for Child Care Staff sponsored by the Department of Education. Staff #4 who had been employed almost 10 months did not have documentation of completion of the Virginia Preservice Training for Child Care Staff sponsored by the Department of Education.
Plan of Correction: Staff #3, #5 and #5 will either locate their certificate of completion if they have taken the Virginia Preservice Training, or they will begin the course and will be asked to complete the 10 hour training within 30 days.
