Inspector Notes
An unannounced inspection was initiated on 3/25/2025 and completed on 4/8/2025 in response to a complaint received by the licensing office on 3/19/2025 relating to allegations of Administration, Personnel, Caregiver Training, Care of Children, and Emergencies. The inspector reviewed 6 children?s records and 5 staff records on-site on 4/1/2025 and 4/8/2025. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Administration and are documented on the violation notice issued to the program.
Standard 8VAC20-800-50-A
Providers must keep a written record of children in attendance each day. There were no attendance records available for the date of 3/17/2025 at the time of the inspection.
Plan of Correction: The licensee stated that many of her attendance records had to be disposed of following an incident where a portion of her documentation became soiled. The licensee produced a completed attendance record for the current date at the time of the inspection on 4/1/2025 and will continue to monitor to ensure that parents sign their children in and out as they arrive and depart. Attendance records will be stored out of reach of children in an area where the caregivers or emergency personnel may easily access it.
Standard 8VAC20-800-50-D
Children's records must be made available to a child's parent upon request, unless otherwise ordered by the court. On 3/19/2025 child #1's parent requested a copy of the incident report for an injury to child #1 that occurred on 3/17/2025. The parent was not provided a copy of the incident report.
Plan of Correction: At the time that the parent asked for a copy of the incident report on 3/19/2025, the licensee had requested to have a meeting with child #1's family and planned to review the incident report with them and provide a copy to the parent at that time. The provider never heard back from the family to schedule the meeting and inadvertently failed to send the requested report. The licensee stated that she will email a copy of the report to child #1's parent today.
