During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 9/9/2025. The on-site inspection began at 9:30 am and ended at 10:50 am. The inspector reviewed compliance in the areas listed above. There was 1 child present and 2 caregivers; the point total was 3. The inspector reviewed 3 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The most recent inspection notice was not posted.
Plan of Correction: Will post most current inspection.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Child #2, enrolled for three months, did not have documentation of a physical examination.
Plan of Correction: Will get documentation from parent.
Standard 8VAC20-800-280
Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children. A bottle of bleach was in an unlocked cabinet below the kitchen sink, accessible to children.
Plan of Correction: Cabinet was locked during inspection.
Standard 8VAC20-800-320-B
Child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Four outlets in the living room and two outlets in the kitchen did not have protective covers.
Plan of Correction: Outlets were covered during inspection.
Standard 8VAC20-800-690-D
The diapering surface shall be cleaned and sanitized after each use. "Cleaned" means treated in such a way as to remove dirt and debris by scrubbing and washing with soap and water or detergent solution and rinsing with water. "Sanitized" means treated in such a way as to remove bacteria and viruses from inanimate surfaces through first cleaning and secondly using a solution of one tablespoon of bleach mixed with one gallon of water and prepared fresh daily or
using a sanitizing solution approved by the U.S. Environmental Protection Agency. The surface of the item is sprayed or dipped into the sanitizing solution and then allowed to air dry. The provider was using clorox wipes to clean and sanitize the surface.
Plan of Correction: Will have cleaning and sanitizing available and labeled.
Standard 8VAC20-800-70-B
The provider shall obtain the parent's written acknowledgement of the receipt of all required items. The record for child #1, enrolled for five months, child #2, enrolled for three months, and child #3, enrolled for fourteen months, information for parents sheet was not filled out with the required information.
Plan of Correction: Will get information from parents.
Standard 8VAC20-800-760-A-1
A first aid kit that contains all required supplies shall be in the family day home, accessible to outdoor play areas, on field trips, in vehicles used for transportation and wherever children are in care. The first aid kit was missing two triangular bandages.
Plan of Correction: Will purchase triangular bandages.
Standard 8VAC20-800-80-A
(Repeat Violation) Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the identity and age of the child. The record for child #3, enrolled for fourteen months, did not contain proof of age and identity.

Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized. Child #2, enrolled for three months, did not have documentation of immunizations on file.

Standard 8VAC20-800-940-B
The provider shall have an operable thermometer available to monitor refrigerator and freezer compartment temperatures. There was no thermometer in the freezer compartment.
Plan of Correction: Will put new thermometer in freezer.
