During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/18/2024, as a part of the licensure period. The on-site inspection began at 9:50am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 16. The inspector reviewed 3 children?s records and 1 caregiver record on-site. This inspection included: document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-120-B
Assistants' records shall contain name, address and telephone number of a person to be notified in an emergency.

Caregiver #1/Assistant record did not contain any emergency contact information.
Plan of Correction: Provider requested information from caregiver and will file once obtained.
Standard 8VAC20-800-200-A
The provider shall orient the substitute provider and assistant by the end of their
first week of assuming job responsibilities.

Caregiver #1/Assistant has provided care for children for almost a month without receiving orientation.
Plan of Correction: Provider will complete orientation with caregiver today.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.
The emergency plan was not reviewed annually, date of last review 8/7/2023.
Plan of Correction: Provider will review/update emergency preparedness plan today.
Standard 8VAC20-800-920
When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated, given to parent or posted, list any substitute food and be kept on file one week at the family day home.

No menus have been created or completed by the provider.
Plan of Correction: Provider to create and post/notify parents of weekly menu.
