Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, and documents review.

A renewal inspection was initiated on 01/14/2022 and concluded on 01/28/2022. The director was contacted by phone and a virtual inspection was conducted of records and a facility tour. There were 89 children present, ranging in ages infant to school-age, with 28 staff supervising and or providing administrative support. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to have an out of state sex offender registry completed for each state an individual has resided in the past 5 years prior to the first day of employment.
Evidence:
1. Staff # 1 (Date of hire: 09/27/2021) did not have an out of state sexual offender registry
completed prior to employment.
2. Staff stated there had been a transition in staff duties and the documentation is not able
to be found.
Plan of Correction: The director will complete the background check and forward the results to the inspector.

Based on record review, the center failed to have a physical exam within 30 days of after
the first date of attendance.
evidence:
1. Child # 5 was enrolled 11/08/21 (DOB 10/09/20) and there is no physical on file.
Plan of Correction: The director stated that the child has a physical examination scheduled for the
week of 01/31/2022.
