During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 4/28/2025. The on-site inspection began at 11:20 AM and ended at 1:30 PM. The inspector reviewed compliance in the areas listed above. There were 9children present and 3 caregivers; the point total was (#). The inspector reviewed 4 children?s records and 3 caregiver/household member(s) records. This inspection included document review, a tour of the family day home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
10Documentation of orientation shall be signed and dated by the provider and caregiver.
Caregiver #3s record did not contain documentation that the provider had completed orientation training with the caregiver.
Caregiver #1 completed 11 training hours in 2024. Caregiver #2 did not complete any training during 2024.
The Supplemental Page from the most recent inspection, containing children and staff information, was observed posted at the family day home.
Child #1 was observed napping without a top cover.
Child #3 (20 months) was observed napping on a cot with a with a pillow.
Child #6 (15 months) and child #7 (10 months) were observed being laid to sleep on their stomachs when placed in their cribs for nap.
Child #5 was observed having a diaper change. The child was placed directly into an activity chair after the diaper change. The childs hands were not washed.
The diaper changing surface was not cleaned and sanitized after child #5 received a diaper change.
The provider has not documented a review of the emergency preparedness plan since 5/29/2023.
Three infant bottles were observed stored in the providers refrigerator that was not labeled with a childs name or todays date.
