Inspector Notes

A initial inspection was initiated and concluded on 11/06/2023. The physical plant, classrooms and materials, and emergency supplies were inspected. 4 staff/agent records were reviewed. No children were present during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, areas and equipment of the center, outside, were not maintained in a safe and operable condition.

Evidence:
1. Visible cracks were observed on four sand table covers located on Playground 1. Peeling paint was observed on the handles of the sand table covers.
2. The coating on the slide and tunnel structure on Playground 1 was peeling and cracked.
3. The covering on the hinge of gate leading to Playground 1 was torn.
4. The door handle on the gate leading to Playground 2 was broken.
5. Peeling paint and rust was observed on the basketball hoop on Playground 2.
