During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on March 26, 2025 from 2:00 p.m. to 4:30 p.m. The inspector reviewed compliance in the areas listed above. There were 33 children present and 6 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-530-A-1
At least one staff in each grouping of children shall have current cardiopulmonary resuscitation (CPR). Staff was alone with 6 two year-olds and Staff 2 was lone with 4 infants. Staff 1 and Staff 2 do not have documentation of current CPR with an in person competency demonstration on file.
Plan of Correction: Staff will get CPR through an organization such as the American Red Cross,
American Heart Association, American Safety and Health Institute, or National
Safety Council that includes an in person demonstration.
Standard 8VAC20-780-560-F-4
Children three years of age or younger may not be offered foods that are
considered to be potential choking hazards. An 18 month old child was served whole baby carrots for snack.
Plan of Correction: Staff will not serve foods that are choking hazards to children under the age of 3.
Standard 8VAC20-780-70
The facility failed to have required documentation in each staff file. When a reference check is taken over the phone, documentation shall include dates of contact, full names of persons contacted, and the firms contacted. References for Staff 2 and Staff 3 did not include dates, full names, and firms contacted.
Plan of Correction: When references are taken over the phone, all required documentation will be noted.
