Inspector Notes
An unannounced, on-site, annual Subsidy Health and Safety Inspection (SHSI) was initiated on 09/20/2024 and completed on 09/20/2024. The on-site inspection began at 10 am and ended at 12 pm. The inspector reviewed compliance in the area listed above. There were (2) children present and (1) caregiver; the point total was (5). The inspector reviewed (4) children?s records and (2) adult household records [on-site] on 09/20/2024. This inspection included:
? document review, policies and procedures, allergy lists (no listed allergies), emergency drill logs, authorization forms;
? tour of the facility; and
? interviews.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and the violations were documented on the violation notice issued to the program.
Violations
6information for each enrolled child, which shall be made available to the department's
representative upon request.
Proof of the child's identity was not available; Name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached; Names of persons other than the custodial parent who are authorized to pick up the child; Written authorization for emergency medical care should an emergency occur; each were not available for Child #1,#2, #3, and #4, all children have been enrolled for over 1 month.

Documentation of the medication policy was not available for review and signed by the parents of Child #1, #2, #3, and #4.
Documentation of the sick policy for Child #1, #2, #3, and #4 was not available for review.
outdoor play areas, on field trips, in vehicles used for transportation, and wherever
children are in care.
A pair of tweezers and a digital thermometer were not available within the first aid kit. This same standard was cited during the initial subsidy inspection.
