Staff and volunteers shall provide a sworn statement or affirmation disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

There was no completed sworn statement or affirmation on file for volunteer 1 who was observed alone with child 2 during the inspection.

Prior to employment or volunteer service staff shall submit to submit to fingerprinting and provide personal descriptive information described in subdivision B 2 of ? 19.2-392.02. The results of the fingerprint background check shall be obtained prior to contact with children.

The results of a fingerprint background check was not on file for volunteer 1 who was observed alone with child 2 during the inspection.

Authorize the child day center, family day home, or family day system described in subsection A to obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 for any founded complaint of child abuse or neglect against him.

A child's record shall contain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

There was no written allergy care plan on file for child 1. Child 1 has a diagnosed food allergy according to documentation from the parent on the enrollment form.
Plan of Correction: File for child 1 will be attached stating that she does not have a food allergy. Application was filled out in September of 2025.

Orientation training for staff shall be completed on the following facility specific topics prior to the staff member working alone with children and within seven days of the date of employment or the date of subsidy vendor approval.

There is no written documentation to demonstrate that staff 1 and staff 2 have completed orientation training. Both staff were present during the inspection and have been employed for more than 17 months.
Plan of Correction: Orientation paperwork will be attached below.

Staff who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training and staff development activities.

There is no documentation to demonstrate that staff 1, 2, 3 and 4 have completed a total of 16 hours of annual training for the past 12 months.

There shall be at least one staff on duty who has obtained within the last three years instruction in performing a daily health observation of children.

There is no written documentation to demonstrate that staff 1, 2, 3, and 4 have completed daily health observation training. These four staff were present during the inspection.
