Inspector Notes

An announced initial Subsidy Health and Safety inspection was conducted with the vendor on October 30, 2023, beginning at 12:47pm and ending at 1:35pm. There were two children, ages 5 months and 4 years old, in the care of the vendor during the inspection for a total of 6 points. Two children's records were reviewed. The vendor and one household member?s records were reviewed and in compliance. The first aid kit, flashlight and smoke detectors were observed and in compliance. There were no medications on sight and allergy care plans to be reviewed.

Not all of the subsidy requirements were able to be accessed during this inspection.

Standard 8VAC20-790-190-B
Based on a review of documentation, the vendor did not obtain all the required information for children enrolled in their program. Evidence: Child A and child B?s records were missing proof of age and identity, written statement of illness policy, and written agreements between parent and vendor.
Plan of Correction: The vendor will obtain the missing documentation from the parent as soon as possible and maintain a record for each child enrolled.

Based on review of emergency supplies, the vendor did not obtain all the required emergency supplies. Evidence: The vendor did not have a battery-operated radio in her emergency supplies.
Plan of Correction: The provider will obtain a battery-operated radio.
Standard 8VAC20-790-420-A
Based on a review of documentation, the vendor did not have a written emergency preparedness plan that plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures. Evidence: The provider did not have a written emergency preparedness plan to address lockdown and shelter-in-place procedures.
Plan of Correction: The provider will create an emergency preparedness plan to address shelter-in-place and lockdown procedures.
