Inspector Notes
A subsidy health and safety inspection (SHSI) was completed on October 7, 2025. The inspection started at 4:05 PM and concluded at 5:33 PM. There were no children present and one caregivers; the point total was zero. The inspector reviewed two children?s records and one caregiver, and two household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

1The tb screening in the record for caregiver #1 was overdue by one month.
