Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.
1. Two high chair covers and 1 bouncy seat's cover were visibly stained and had old food particles on them.
2. One large foam floor mat had visible tears.
3. One cloth floor activity mat for children was torn around the edges exposing the inner stuffing, making it a choking hazard.

Plan of Correction: The floor mat and activity mat will be removed. The covers for the high chairs and bouncy seat will be washed and cleaned.
