Inspector Notes

An unannounced inspection was initiated on 10/17/2024 and completed on 10/29/2024 in response to a complaint received by the licensing office on 10/16/2024 relating to allegations of behavioral guidance. The inspector reviewed 6 children?s records and 5 staff records on-site on 10/29/2024. This inspection included document review, a tour of the facility, interviews and video observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Various cleaning agents were observed in an unlocked cabinet in the infant classroom.
Plan of Correction: We will add this to our checklist for our daily walk through of the school.
Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Video footage of nap time in the toddler classroom on 10/15/2024 showed staff #4 yelling at a child to sit down on their cot while staff #4 sat on the floor patting the back of another child across the classroom. Staff #4s behavioral guidance was not age and stage appropriate.
Plan of Correction: We will approach and coach the teachers. We will continue to monitor the teachers in the toddler classroom.

Cots shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Two out of thirteen children in the pre-k classroom did not have a bottom sheet on their cot during naptime.
Plan of Correction: We keep extra sheets in the classrooms. We will coach and remind the teachers to use the extra sheets if a child does not bring them in.

Infant bottles shall be dated and labeled with each childs name.

Two out of two bottles in the infant classroom were not labeled with a name or date.
Plan of Correction: The parents should be labeling at home, but we will make sure as soon as a child arrives that the staff are checking.
