Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.

On 10/28/2025, staff #1 grabbed and pulled on child #1 (age 3 years) arm and dragged them approximately 18 feet because the child was touching a chair that was close to their cot. Child #1?s left elbow was dislocated as a result of the incident and was observed crying.
Plan of Correction: Staff received additional training on Heath & Safety, Teacher sensitivity and understanding of Nurse Maid's Elbow to ensure safe and appropriate physical interactions with Children.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities and services are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards.

The center?s discipline and guidance policy states that ?discipline must be: directed toward teaching the child acceptable behavior and self control.? The center?s discipline and guidance policy also states ?The following types of discipline and guidance are prohibited: picking up or lifting a child by the hands or arms.?

Staff #1 did not follow the center?s policies when they grabbed and dragged child #1 (age 3 years) by the arms as punishment for putting their mouth on a chair during nap time.
Plan of Correction: 1. Mandatory Policy review: All staff members will participate in a Behavior guidance and discipline policy with specific emphasis on "Forbidden Actions."
2. Increased frequency of classroom monitoring by the Admin staff.
3. Staff will sign an updated acknowledgment confirming their understanding of prohibited disciplinary actions.
Standard 8VAC20-780-400-B
REPEAT VIOLATION
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Based on video footage obtained from the center, on 10/28/2025, child #1 (age 3 years) was observed moving a chair that was against the child?s cot while the class was waking up from naptime. Staff #1 was observed saying ?Hey hey, what are you doing? Go back to your bed [child #1], stop moving the chair. On your cot. On your cot!? Child #1 was observed quickly laying down on their cot once staff #1 started to approach the cot. Staff #1 was observed saying ?You think it?s funny? You don?t want to sit there and think it?s funny? Come with me, get up. Get up.? Staff #1 was observed lifting child #1 by their left forearm from lying on the cot to a standing position. Staff #1 was observed telling child #1 ?Walk. Walk. Walk. Let?s see how funny now. It?s funny now?? as child #1 cried.
Plan of Correction: Staff will complete refresher training on Positive Behavior Guidance, Conflict resolution and Age appropriate redirection strategies by December and new hire orientation will now include training on constructive, age appropriate guidance & redirection techniques.
Standard 8VAC20-780-410-1
The following acts are forbidden: roughly handling.

Plan of Correction: A certified trainer from Foundation First was brought in to provide staff training. Staff required to complete regular competency quizzes that reinforce safe handling & lifting practices.
