Inspector Notes

An unannounced monitoring inspection was initiated and completed on June 24, 2025 in response to a self-report received by the licensing office on June 18, 2025 relating to supervision. The inspector reviewed 5 children?s records and 1 staff record on-site. This inspection included document review, observations, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to ratios and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 10, 2025. A POC submitted after this date will not appear on the public website.

Staff-to-children ratios are required whenever children are in care.

On June 24, 2025, there was a group of 11 children, including two year olds, with one teacher exceeding the 1:8 required ratio. The classroom was out of ratio for approximately 6 minutes.
Plan of Correction: The child to adult ratio was corrected during the inspection and maintained throughout today. The state adult to child ratio shall be maintained at all times including transition times, and rest time.
