During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated on September 17, 2025 and completed on September 17, 2025, as a part of the provisional licensure period. The on-site inspection began at 10:30am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver the point total was 11. The inspector reviewed 3 children?s records and 2 caregiver/household member(s) records on-site.and electronically on September 17, 2025.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 29, 2025. A POC submitted after this date will not appear on the public website.
Violations
5Child 2 has been enrolled for 13 months without documentation of a physical.
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Caregiver 1 has completed fifteen hours of training.
Caregiver 2 has completed seven hours of training.
A child's record shall contain all required information.
Child 2's record did not contain a health insurance policy number.
Child 3's record did not contain the Child's enrollment date; the names, phone numbers and work information of the child's parent's; the names, addresses and phone number's of two emergency contacts; the name and policy number of the child's heath insurance; and the name and contact information of the child's physician.
The diapering surface was not cleaned and sanitized after one of the two observed diaper changes.
Before the child's first day of attendance, parents shall be provided in writing all required information.
Child 2's record did not contain documentation that parents were provided the provisions of the emergency plan.
Child 3's record did not contain documentation that the parents were informed whether the provider carries liability insurance.
