Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Evidence: The record for Staff #3 contained an expired tuberculosis screening. The screening expired seven days ago.
Plan of Correction: Staff member was directed to obtain a TB test the same day. Results were received and filled immediately. A digital tracking system with 30-day alerts is now in place for all staff records.

Areas and equipment of the center inside and outside shall be maintained in a clean, safe, and operable condition.

Evidence:

1. In the Preschool 4 class, ages four and five-years old, one of the two toilets was clogged and contained feces, the water in the toilet had turned brown and a thin brown film formed over the top of the water. The teacher was unaware that the toilet was clogged and could not say how long it had been in this condition.

2. In the Preschool 4 class, chipping and peeling paint was observed in the block and library area.
Plan of Correction: Toilet in Preschool 4 was immediately plunged and sanitized. Staff retrained on hourly restroom checks. Chipping paint in block/library area will be sanded and repainted with non-toxic paint.

The staff-to-children ratio of 1 staff to four infants (birth up to 16 months) are required whenever children are in care.

Evidence: In the infant class, ages birth ? 12 months, nine children were present with two staff.
Plan of Correction: The ninth child was a staff member's child visiting the room. The child was immediately removed to restore the 1:4 ratio. Staff have been retrained on the policy that visiting children must not impact state ratios.

Shelter in place procedures shall be practiced a minimum of twice per year.

Evidence: The Emergency Report form shows only one shelter in place drill was conducted in 2025. The last documented shelter in place was dated 3/14/25, the drill is four months overdue.
Plan of Correction: A shelter in place drill will be conducted by 02/25/2026. Master calendar updated to include mandatory drills every six months.

Documentation of two or more references as to the character and reputation as well as competency are required to be checked prior to employment.

Evidence: The record for Staff #4 did not contain references. Staff #4 has been employed for four months.
Plan of Correction: Management contacted references for Staff #4. Documents were placed in personnel file. New Hire Checklist Implemented for Director sign-off before start dates.
