Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on January 30, 2025, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:50am. The inspector reviewed compliance in the areas listed above.There were 38 children present and 12 staff. The inspector reviewed six children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The center must request a search of the central registry prior to the employee's first day of employment.

Staff #1, Staff #2, and Staff #4 did not have a central registry search requested. Staff #1 has been employed for over a month. Staff #2 and Staff #4 have been employed for over two months.
Plan of Correction: Management will contact OBI and document the record for any follow up with Central Registry searches. Central Registry searches will be completed prior to employment.
Standard 8VAC20-780-140-A
Each child shall a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.

Child #3 does not have documentation of a physical examination in their record. Child #3 has been enrolled for over two months.
Plan of Correction: Management has contacted the parent to obtain a copy of a physical examination.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment.

Staff #4 and Staff #5 have documentation of a negative tuberculosis screening not dated within the last 30 days of the date of employment. Staff #4's tuberculosis screening was dated more than 60 calendar days of the date of employment. Staff #5's tuberculosis screening was dated more than 60 calendar days of the date of employment.
Plan of Correction: Moving forward, the center will ensure that TB screenings are completed at the time of employment or within the last 30 calendar days of the date of employment.
Standard 8VAC20-780-60-A-8
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #6 has a diagnosed food allergy. Documentation of a written care plan was not available for review.
Plan of Correction: Management contacted the parent and obtained the allergy care plan by email and documented the record on the day of the inspection.
