Inspector Notes

An unannounced inspection was initiated and completed on March 31, 2025 in response to a complaint received by the licensing office on March 27, 2025 relating to allegations of forbidden actions and special care provisions and emergencies. The inspector reviewed three children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Background Checks and are documented on the violation notice issued to the program.

Repeat Violation

The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, who had been employed for four months, did not have a documentation that central registry search was requested and search results were not available at the time of the inspection.
Plan of Correction: Staff #1's central registry search has been completed and resubmitted to the portal. Results will be sent to the licensing inspector when returned. Moving forward, management will ensure to conduct a central registry search for staff prior to employment and follow up with OBI when results are not returned in a timely manner. Documentation of this follow up will be recorded in the staff record.
