Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 24, 2025, as a part of the licensure period. The on-site inspection began at 10:40am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 68 children present and 18 staff. The inspector reviewed five children?s records and seven staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There were no physical examinations in the records for Child #3 and #4. Child #3 has been enrolled for eight months and Child #4 has been enrolled for two months.
Plan of Correction: The parents have been contacted to obtain the physicals for the children. The center will ensure to obtain all physicals at the time of enrollment or 30 days after the first day of enrollment.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

The battery operated storage system for diapers located in Classroom #6 and Classroom #7 were not operational. The batteries for both storage systems for diapers were no longer functioning.
Plan of Correction: Batteries were placed in the battery operated diapering receptacles at the time of the inspection.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

One bottles of prepared formula was not labeled with a date and the child's name.
Plan of Correction: The bottles was labeled with the child's name and date at the time of the inspection. Staff will ensure that all bottles are properly labeled upon arrival.

Staff records shall be kept for each staff person with required information.

The name, address and telephone number of a person to be notified in an emergency was missing from Staff #1's record. Staff #1 has been working for one month. Documentation to demonstrate the individual possesses the education, certification, and experience required by the job position was missing from Staff #4's record. Staff #4 has been working at the center for five months.
Plan of Correction: The emergency contact for Staff #1 was obtained the day of the inspection and documentation of education will be placed in Staff #4's record by tomorrow.
