Inspector Notes

An unannounced inspection was initiated on 7/28/2025 and completed on 8/4/2025 in response to a complaint received by the licensing office on 7/24/2025 relating to allegations of physical plant, supervision and infant sleeping standards. The inspector reviewed 1 staff records on-site. This inspection included: document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid.
Though not a part of the original complaint, violations that were not reported were found related to infant feeding and staff records and are documented on the violation notice issued to the program.

Standard 8VAC20-780-240-C
Orientation training shall be appropriate to the age of the children in care and include all of the following facility specific topics: the center's emergency preparedness plan, prevention of and response to emergencies due to food and other allergic reactions, prevention and control of diseases.
Staff A's record contained incomplete documentation of orientation training. Orientation documentation did not include; the center's emergency preparedness plan, prevention of and response to emergencies due to food and other allergic reactions and prevention and control of diseases.
Plan of Correction: When Center was initially applying for licensure our orientation document was submitted and approved. However, after this has been brought to our attention, all staff have a now been given an updated orientation training that includes these additional topics.

Bottle fed infants who cannot hold their own bottles shall be held when fed.
An infant was observed lying on a "boppy pillow" on the floor with a staff member also sitting on the floor, bottle feeding the child.
Plan of Correction: All infant room teachers have been retrained to understand infants who cannot hold bottles shall be held during the feedings.
