During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 02/10/2026 and completed on 02/10/2026. The on-site inspection began at 9:05 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 79 children present and 17 staff. The inspector reviewed 8 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 02/25/2026. A POC submitted after this date will not appear on the public website.

The center must obtain results of a central registry request by the 30th day of employment.

Staff #3, who had been employed for over four months, did not have documentation of central registry results.
Plan of Correction: Obtained and copy attached placed in filed.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry, criminal history, or sex offender registry from one state for Staff #5, who has been employed for more than two months.
Plan of Correction: Once backgrounds requested follow/ups. Attached copy of background check follow up and document.
Standard 8VAC20-780-240-C
Orientation training shall be appropriate to the age of the children in care and include all of the following facility specific topics: Recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse.

Staff #5 has been employed for over 2 months and did not have documentation of recognizing child abuse and neglect and the legal requirements for reported suspected child abuse.
Plan of Correction: The staff member completed the training and attached certificate. Will double check training to avoid repeated violations.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Child #3 did not have the address listed for emergency contacts.
Plan of Correction: Parent provided emergency contact address of neighbor. Double check enrollment form.
