Inspector Notes

An unannounced complaint inspection was initiated with the director on July 3, 2024 beginning at 10:26 AM and ending at 11:33 AM. The inspection was conducted to investigate an allegation regarding supervision, behavioral guidance, forbidden actions, and handwashing and toileting. Facility record review and interviews were conducted. The investigation was concluded on July 18, 2024. Based on the preponderance of evidence, the complaint was determined to be not valid.
