Inspector Notes
An unannounced monitoring inspection was initiated and completed on February 14, 2024, in response to a self-report received by the licensing office on February 14, 2025 relating to forbidden actions. The inspector reviewed 1 children?s records and 2 staff records on February 14, 2025. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations (this would include observations of photos, recordings, etc.).
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to (area or part of standard) and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/28/2025. A POC submitted after this date will not appear on the public website.
Violations
2Staff #1 leaned over child #1 in an intimidating manner and yelled in his faces to correct behavior.
On February 14, 2025, Staff #1 roughly grabbed Child #1 by the wrist and pulled away from the cubby area.
