Inspector Notes

An announced initial inspection was conducted with the administrative staff from 8:45 am to 11:55 am. Measurements of the inside and outside spaces were taken. There was ample supplies, furnishings and equipment for the requested capacity. Many of the standards could not be determined as the center has not opened and there are no children enrolled. Your temporary Certificate of Occupancy (CO) expires on 7/5/24. You must submit an up-to-date CO prior to 7/5/24 to continue to operate. Please submit the additional policies and procedures by 3/8/24. The area of non compliance is outlined on the violation notice.

Based on observation and measurement, it was determined that there was an opening between the fence and gate post on the older playground that allowed a 3.5 inch by 6 1/4 inch rectangle to fit throught but did not allow a 9 inch circle to fit through, therefore resulting in a head entrapment hazard.
Plan of Correction: The entrapment hazard on the fence was corrected.
