An unannounced monitoring inspection was initiated on 5/28/25 and completed on 6/11/25 in response to a self-report received by the licensing office on 5/16/25 relating to the program. The inspector reviewed 1 child's records and 1 staff record on site. This inspection included document review, tour of the facility, interviews, and observations.
