Inspector Notes

An unannounced inspection was initiated on 8/27/2025 and completed on 9/5/2025 with a local agency in response to a complaint received by the licensing office on relating to allegations of Staffing and Supervision, Programs and Special Care Provisions and Emergencies. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/15/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 8/21/2025, video footage shows Child A (age 18 months), was able to gain access to a soiled item in a plastic bag behind the changing table. Child A was seen carrying the plastic bag for approximately 5 seconds, until a staff notices, and removes the bag from the child?s hands.
Plan of Correction: Staff were retrained on classroom positioning to ensure proper supervision.
Standard 8VAC20-780-500-A
Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.

Child A?s hands were not washed after coming in contact with a bag that contained a soiled item.
Plan of Correction: Staff were retrained on proper handwashing. Child A's parent was present during this time.
