An unannounced inspection was initiated on 11/14/25 and completed on 1/15/26 in response to a complaint received by the licensing office on 11/10/25 relating to allegations of a forbidden actions and handwashing. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
