When staff are supervising children, they shall always ensure their care, protection, and guidance.

1. On 1/20/26 at approximately 12 noon, child A (2 years of age) repeatedly climbed on and jumped off a bookshelf in the classroom. Staff on more than 3 occasions told the child from across the room to discontinue the jumping and climbing on the bookshelf. The child continued the behavior and fell and sustained a gash across their right eyebrow. 911 was called and the child received six stitches.
2. There were 2 staff in the classroom with 12 children. Neither staff approached the child in an attempt to stop or redirect the behavior.
Plan of Correction: During the transition to rest time, one teacher was completing a diaper change while the other was cleaning up after lunch. The children were moving to their mats for nap. A child was repeatedly reminded by the teacher at the changing table to lie down. After finishing the diaper change and washing her hands, the teacher immediately went to assist the child who was jumping. Both teachers
have since been retrained on active supervision and naptime procedures to ensure children are consistently monitored during transitions.
