Inspector Notes

A monitoring inspection was initiated and concluded on 02/14/2024. The inspector arrived at 9:30 AM and departed at 10:45 AM. There were 3 infants present with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the administrator during the exit interview.

Standard 8VAC20-780-270-A
Based upon observation, the facility has not ensured that areas and equipment of the center are safe.
Evidence:
There was water-filled bottle warmer on a shelving unit beside the diaper changing table. the cord from the bottle warmer was hanging loose between the shelf and the changing table, allowing for the possibility that a child could pull the bottle warmer off of the shelf onto him/herself.
Plan of Correction: The facility responded with the following:
The bottle warmer will be moved to the food preparation area with the cord secured out of reach of the children.
Standard 8VAC20-780-280-B
Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
1. The safety clasp on storage area under the diaper changing station was not latched. There was a spray bottle of bleach/water sanitizing liquid stored in this unlocked area under the changing table.
2. There was a tube of diaper ointment with a warning label to include "keep out of reach of children" in an open bin on the low storage shelving unit beside the changing table.
3. The door to the laundry room does not have a lock or child proof latch. There was laundry detergent, disinfectant spray and multi-purpose cleanser, all with warning labels, stored in this room.
Plan of Correction: The facility responded with the following:
All staff will be reminded that the child proof latch on the cabinet under the changing table must be kept latched at all times.
Staff will be reminded that all ointments, creams and any other product labeled warning, caution, hazardous, keep out of reach of children, etc. must be kept in locked locations or areas with child proof latches.
A child proof latch will be purchased and placed on the door to the laundry room to ensure the children cannot access the room.
Standard 8VAC20-780-340-C
Based upon observation and staff interview, the facility has not ensured that during the stated hours of operation, there is always on the premises when one or more children are present, one staff who is the program leader and another immediately available second staff person.
Evidence:
Upon arrival of the inspector at 9:30 AM, staff 1 was on duty by herself with three infants. When asked, staff 1 stated that she is usually the only staff person on duty in the facility.
Staff 2 arrived and stated that she covers breaks for staff 1, but is not at the facility unless needed to cover for staff 1.
Plan of Correction: The facility responded with the following:
At least two staff will be on duty at all times in the infant facility. All staff will be reminded that two staff must be present at all times.
