During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 03/24/2025, as a part of the licensure period, The on-site inspection began at 10:50 AM and ended at 12:40 PM. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 staff. The inspector reviewed 8 children?s records and 3 staff records on-site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
5An infant was in care for nine months without documentation that the child received the required immunizations.
The parent was contacted today and will be required to bring in the child's physical examination. All children's records will be checked to ensure that they included the results of a physical examination signed by a physician.
A fifteen month old child attended for nine months without any immunization updates.
A tracking system will be used to ensure that updated immunization records are obtained every six months for children under two years.
A well baby check for an infant in care for 8 months did not have the child's name and was not signed by a physician.
There was paper litter scattered throughout the infant outdoor play space.
All staff will be reminded to check that the outdoor infant play space is maintained to be free of litter and debris and should be checked daily before the infants are taken outside.
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
There were nine children in care in the infant room but only 8 children had been added to the attendance record.
All staff will be reminded to add each child to the attendance sheet as they arrive and remove them from the attendance when they are picked up each day.
