Inspector Notes

An unannounced monitoring inspection was completed on 01/14/2025 as a part of the licensure period. The on-site inspection began at 1:00pm and ended at 2:15pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 14 children present and4 staff. The inspector reviewed 4 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The center did not have documentation that Child #1 received the required immunizations required by the State Board of Health.
Plan of Correction: The director will contact the parents to obtain documentation of physicals and immunization records that meet all requirements.
Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician

Child #1 did not have documentation of having physical examination by or under the direction of a physician.
Plan of Correction: The director will contact the parents to obtain documentation of physicals and immunization records that meet all requirements.
Standard 8VAC20-780-150-B
Immunizations and physical examination reports for children shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

In the infant room there was a protruding piece of wood at the base of the floor in reach of infants. It was rough and jagged to the touch. There were also two cribs had mattress sheets that were loose and did not fit snug to the mattress posing a suffocation hazard.
Plan of Correction: The owner has already purchased the materials to cover the protruding wood and plans to install it by next week. The owner will also ensure that all crib sheets fit snug to the mattress.
