Inspector Notes
An unannounced monitoring inspection was completed on 01/14/2025 as a part of the licensure period. The on-site inspection began at 1:00pm and ended at 2:15pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 14 children present and4 staff. The inspector reviewed 4 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
4The center did not have documentation that Child #1 received the required immunizations required by the State Board of Health.
Child #1 did not have documentation of having physical examination by or under the direction of a physician.
Child #1 did not have documentation of an immunization report signed by a physician, his designee, or an official of a local health department.
In the infant room there was a protruding piece of wood at the base of the floor in reach of infants. It was rough and jagged to the touch. There were also two cribs had mattress sheets that were loose and did not fit snug to the mattress posing a suffocation hazard.
