Inspector Notes

An unannounced monitoring inspection was completed on 09/04/2025, as a part of the licensure period. The on-site inspection began at 10:40am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements.There were 10 children present and 4 staff. The inspector reviewed 2 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. The bottom drawer of the changing table was not locked. Inside were 2 cans of disinfectant spray/
Plan of Correction: The safety lock on the drawer had just broken. The chemicals were removed to a locked place during the inspection.
Standard 8VAC20-780-340-D
In each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present. Such a program leader shall supervise no more than two aides. Staff #2 did not have documentation that she met the qualifications of a program leader. Staff #2 was observed in the role of program leader with 6 children between 2.5 and 3 years of age and 2 aides.
Plan of Correction: Normally the director is in the class, however that day she had left the center to buy supplies. The Director is working on qualifying staff #2 as a program leader and plans to have the training complete by 09/25/2025
Standard 8VAC20-780-520-C
If diaper ointment or cream is used written parent authorization noting any known adverse reactions is required. There were 1 diaper ointment that did not have written authorization and another diaper ointment that authorization that had expired.
Plan of Correction: The director went through all diaper ointments and obtained written authorizations for the children currently in care and discarded any others.

When food is brought from home the food container shall be clearly dated and labeled in a way that identifies the owner. There were 6 lunch boxes/bags that were not labeled with the date.
Plan of Correction: After the inspection tags were placed on the children's lunch boxes to easily label the dates each day.
