Inspector Notes

A monitoring inspection was initiated and concluded 03/20/2024. The inspector arrived at 12:30 PM and departed at 2:12 PM. There were 3 children present, totaling 6 points, with the provider and an assistant supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 provider/assistant records were reviewed.

Information gathered during the inspection determined a non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. Violations were reviewed with the provider during the exit interview.

Based upon review of children's records, the provider has not ensured that each child's record includes the name, address and telephone number of each parent who has custody.
Evidence:
The record provided for child 3 did not include the name, address and telephone number for the father. No custody papers were in the record.
