During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 07/21/2025, as part of the licensure period.The on-site inspection began at 9:00 AM and ended at 11:10 AM. the inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers. The point total was 14. The inspector reviewed 6 children's files and 3 caregiver/household records on site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on July 28. A POC dated after this date will not appear on the public website.
Violations
3The immunization record provided for child 5 was not signed by a licensed physician, the physician's designee or an official of a local health department..
Mom explain she use my chart to printher daughter shot records out to save time & provided us with a signed shot record.
Each child's record includes all required information.
The enrollment record completed by the parent of child 1 and the physical signed by the physician had an incorrect date of birth.
The records provided for child 1 and child 3 did not include the street addresses for the emergency contact persons other than the parent(s).
Mom corrected her mistake on the enrollment packet and provided us with an updated physical.
Mom provided the address to her emergency contacts.
The records provided for all enrolled children did not include written parental permission for the children to be taken off the licensed premises for walks in the neighborhood.
We obtain signatures from the parent. The parent also agreed to the daily morning walks.
