Inspector Notes
An unannounced inspection was initiated on October 1, 2024, and completed on October 8, 2024 in response to a complaint received by the licensing office on September 9, 2024 relating to allegations in the areas of Supervision and Programs. The inspector reviewed five children?s records and seven staff records on site on October 2, 2024. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 17, 2024. A POC submitted after this date will not appear on the public website.
Violations
2Staff #1 was present in the two-year-old classroom and videotaped Child #1 as he slapped Child #2 in the face two times. Staff #1 did not intervene to prevent an injury to Child #2 who sustained a bruise on the face.
It was reported by the parent that Child #1 was seen sitting in a high chair while other children were up moving playing and dancing around the room. Multiple staff report that Child #1 was sometimes placed in a high chair to retrain him from moving freely around the classroom.
