During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 06/11/25 and completed on 06/11/25, as a part of the licensure period. The onsite inspection began at 9:45am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 55 children
present and 11 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The most recent fire inspection on file for the center was conducted on 10/23/23.
Plan of Correction: A fire inspection will be conducted by the appropriate authority in the coming weeks.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 .

The plastic slides and climbing equipment on the playground did not have at least six feet of resilient surfacing on all sides as required.
Plan of Correction: Additional resilient surface will be added where required.
Standard 8VAC20-780-340-D
In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Staff #2 was observed alone in the school age classroom and was identified as a program leader for the group. Documentation reviewed and interviews conducted do not support that Staff #2 meets qualifications of a program leader.
Plan of Correction: Staff #2 will complete additional training to ensure they meet qualifications as a program leader.
Standard 8VAC20-780-350-B-1
The required ratio for children birth up to 16 months is one staff per four children.

Three staff were observed with 12 children in the infant classroom. During the inspection of the classroom, staff were observed walking out of the classroom and into the diaper changing room, leaving two staff alone with 11 children, approximately three times.
Plan of Correction: There is always additional staff available to step in during diaper changes. Staff were reminded to contact additional staff when needed.
Standard 8VAC20-780-500-B
A non-absorbent surface for diapering or changing shall be used.

One of the diaper changing pads in the infant classroom had a tear and was no longer considered non-absorbent. Staff were observed changing diapers on the changing pad.
Plan of Correction: New changing pads were ordered. The center will not use non-absorbent changing pads during diaper changes in the future.
