Inspector Notes

An announced initial inspection was conducted with the owner from 9 am to 11:15 am. The inside and outside spaces were inspected and measured. Four staff files were reviewed.

The supplies and furnishings appear sufficient for the requested capacity. Three cribs were inspected and met the CPSC guidelines ( manufacture date-6/2023). At this time the center is not planning to administer medication and the children's meals will be catered.

Please make the required changes to your policies and procedures and resubmit by 3/8/24.
Please submit documentation that you have at least two staff who have obtained CPR certification.

Standard 8VAC20-780-160-A
Based on interview and a review of four staff files, it was determined that all staff had not obtained tuberculosis test/screenings at the time of employment.
Evidence:
Staff # 3 and 4 did not have tuberculosis test/screenings on file.
Staff # 3 was hired on 12/15/23 and staff #4 was hired on 1/9/24.
Plan of Correction: We will have the staff get the tuberculosis tests/screenings.
Standard 8VAC20-780-430-C
Based on observation and measurements, it was determined that the two composite play structures on the playground presented head entrapment hazards.
Evidence:
There were side openings on the two play structures that allowed a 3-1/2 inch by 6- 1/4 inch rectangle to fit through but did not allow a nine-inch circle to fit through, therefore presenting a head entrapment.
Plan of Correction: We will close the openings to ensure that they do not present head entrapment hazards.
Standard 8VAC20-780-530-C
Based on interview and a review of four staff files, it was determined that the center did not have at least two staff who had CPR certification to include an in-person competency demonstration.
Evidence:
Staff #1, 2, 3 and 4 did not have CPR certification to include an-in person competency demonstration.
Plan of Correction: We will have the staff obtain the in-person CPR training.
Standard 8VAC20-780-540-E
Based on observation and interview, it was determined that there was no working battery operated radio on site.
Plan of Correction: We will get new batteries for the radio.
