Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
There was makeshift padding attached to the headboard of two cribs with a rope.

Repeat Violation:
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
There was insufficient resilient surfacing ( mulch) surrounding the two climbing structures and slides. The mulch in various places had degraded to soil, which is not an acceptable surfacing, in some places the mulch was compact and not resilient and other places the mulch was between 2 to 4 feet in depth and not the required 6 feet of mulch.

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
Child A has an allergy to dairy, however there was no written list identifying the allergy in the child's classroom. This information was missing from the classroom for 6 months.
