During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 05/27/2025 and completed on 05/28/2025. The on-site inspection began at 10:10 AM and ended at 12:12 PM. The inspector reviewed compliance in the areas listed above. There were 26 children present and 5 staff. The inspector reviewed 5 children?s records, 9 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff # 1, who had been employed for four months, had a national criminal background check dated 23 days after hire.

Staff # 4, who had been employed for four months, had a national criminal background check dated eight days after hire.

Staff # 6, who had been employed for three months, had a national criminal background check dated 16 days after hire.

Staff # 9, who had been employed for four months, had a national criminal background check dated nine days after hire.
Plan of Correction: The director in the future will ensure that all Criminal Background Checks are completed prior to hire.

Prepared baby food not consumed during that feeding by an infant may be used by that same infant later in the same day, provided that the food is not served out of the baby jar and is dated and stored in the refrigerator; otherwise, it shall be
discarded or returned to the parent at the end of the day.

Child # 5 had an open container of baby food that was not dated.
Plan of Correction: The director will ensure that staff label the containers of baby food and will monitor.

Documentation of at least 2 references to character, reputation, and competencies are to be checked prior to employment.

Staff #6, who has been actively working at the center for the past 3 months, had one of the two required references on file.
