Inspector Notes

An announced initial subsidy health and safety inspection was conducted on April 26, 2024 from approximately 9:50 am to 12:15 pm. There were 18 children in attendance and a total of 5 staff present. 9 staff records and 5 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed playing outside and getting ready for lunch, while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please specify how the deficient practice will be or has been corrected. Based on record review, the vendor failed to ensure that staff shall submit documentation of tuberculosis (TB) screening that was completed within the last 30 calendar days of the date of employment.

Based on observation, and interview, the vendor failed to provide a cot, rest mat, or bed for children older than 12 months of age.
Evidence: Classroom #2 contained a portable "pack-n-play". Interview with staff revealed that child #4, age 2 years 2 months, takes a nap in the "pack-n-play" daily.
