Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/30/2026. A POC submitted after this date will not appear on the public website.
Violations
2On February 23, 2026, staff #1 admitted to hitting child #1 (age 3) on the bottom due to being frustrated the child would not listen to redirection during rest time.

redirection, positive reinforcement, and age-appropriate conflict resolution. Training also reinforced that all guidance must be constructive, non-physical, and supportive of children?s developmental needs. Additionally, the center?s discipline policy was reviewed in full with all staff, with emphasis on zero tolerance for any form of physical punishment.
On February 23, 2026, Staff #1 admitted to hitting child #1 (age 3) on the bottom three times with a closed hand. One staff in a different room heard child #1 cry out after being hit. Staff #1 admitted to two staff that they hit child #1 on the bottom.
of the incident, immediate action was taken, including termination of the staff member involved. All staff have since been retrained on licensing standards that explicitly forbid physical punishment, including striking a child under any circumstances. Staff have signed acknowledgment forms confirming their understanding of the center?s discipline policy and the prohibition of physical punishment.
