During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Inspector Notes

An unannounced monitoring inspection was initiated on 3/5/2026 and completed on 3/13/2026 with a local agency in response to a self-report received by the licensing office on 2/24/2026 relating to supervision. The inspector reviewed 1 child?s record and 3 staff records on-site. This inspection included tour of the facility, document review, observations and interviews.

Information gathered during the inspection determined non-compliance with applicable code sections, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/30/2026. A POC submitted after this date will not appear on the public website.

The center shall establish and implement procedures for: Appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children.

The center?s policy states: ?Teachers use praise, guidance, redirection, modeling, and setting clear limits to lead children to make positive choices and develop successful interactions with others. In some situations, teachers may use age-appropriate time out, a strategy which gives a child the opportunity to step out of a situation, breathe, reflect, and talk calmly with the teacher about the problem.?

On February 23, 2026, staff #1 admitted to hitting child #1 (age 3) on the bottom due to being frustrated that the child would not listen to redirection during rest time.
Plan of Correction: Upon notification of the incident, the staff member involved was immediately
terminated. This action ensured the immediate safety of all children in care and removed any risk of recurrence involving that individual. The center reviewed the incident in full to identify breakdowns in supervision and staff response. As a result, supervision expectations and procedures have been reinforced with all staff. All staff have been retrained on active supervision requirements, appropriate responses to challenging behaviors, use of redirection and de-escalation techniques, and the requirement to seek assistance when feeling overwhelmed. Staff were reminded that physical discipline is strictly prohibited.
