Inspector Notes
An unannounced, on-site inspection was initiated on August 1, 2025 completed on August 1, 2025 in response to a complaint received by the licensing office on July 23, 2025 relating to allegations of Staffing and Supervision, Programs, and Special Care Provisions and Emergencies. This inspection included tour of the facility, interviews and observation.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
