An unannounced monitoring inspection was initiated on 9/18/2024 and completed on 10/8/2024) in response to a self-report received by the licensing office on 9/18/2024. The inspector reviewed 1 staff record electronically on 9/24/2024. This inspection included document review, interviews, and observations.

"The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.
On 9/18/24, Staff #1 pulled Child #1 (age 3 years) by their hair when they were acting out.

Plan of Correction: Staff #1 was removed from the classroom and subsequently dismissed from employment. Ongoing trainings on proper handling of children as well as how to deal with stressful situations in the classroom.
