During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/27/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-520-F
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present. The allergy list in one of the rooms was not dated.

Standard 8VAC20-790-540-A
The center shall maintain and keep at the center a record for each enrolled child that contains all of the required information. The record for child #2 did not include emergency contacts. The record for child #4 did not include documentation of immunizations or proof of identity.
Plan of Correction: All students records are being reviewed, corrected and updated
Standard 8VAC20-790-550-5
Staff files shall include documentation that orientation training has been completed. There was no documentation of orientation in the 5 staff files that were reviewed.
Plan of Correction: All employee files will list: Hire Date, Start Date, Orientation Date, signed Employee Handbook Agreement of Terms and signed Staff Orientation List.
Standard 8VAC20-790-560-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 TB was dated 20 days after employment, Staff #2 TB was dated 3 months after employment, Staff #3 TB was dated 8 days after employment, Staff #4 TB was dated 2 months after employment, and Staff #5 TB was dated 2 days after employment.
Plan of Correction: TB screenings will be set up at time of hire with the Dept of Health by the HR dept.

Within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff. Staff #2 completed the training 4 months after their employment.
Plan of Correction: Staff will complete the Virginia Preservice Training for Child Care during the orientation period.
Standard 8VAC20-790-640-A
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be
kept in a locked place using a safe locking method that prevents access by children. Hazardous chemicals and sanitizer were observed, unlocked, in 5 of the 9 rooms children were occupying.
Plan of Correction: All cleaning bottles have been removed, all baskets have been taken down and new procedure has been written.

Evacuation procedures shall be practiced at least monthly. There were no practice drills documented for the months of May, July, August, September, October, November, and December of 2024.
Plan of Correction: Emergency Drills to include Shelter in Place and Lock Down to be conducted as per Va Standards.
