Inspector Notes
An unannounced monitoring inspection was conducted today between the hours of 9:00 am through 10:30am. There were a total of five children (15 points) observed with one caregiver. A total of five children's files and one staff/household member's files were reviewed. The children were engaged in the following: toy play, games, tummy time, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. 2.Child #2's record (Start date-01/08/2024) did not contain documentation of a physical examination.
3.Child #3's record (Start date- 11/11/2023) did not contain documentation of a physical examination.
4.Child #4's record (Start date- 11/16/2023) did not contain documentation of a physical examination.
5.Caregiver #1 confirmed that Child #1, Child @, Child #3, and Child #4 did not have documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
